cheering for the waddlers, or volunteering)

 ONLY ONE ATTENDEE may be signed up at a time. Additional

attendees must be signed up separately.

Liability Release

Assumption of Risk. I know that participating in a fun run or race, even when walking, is a potentially hazardous activity. I assume all

risks associated with the Activity including, but not limited to, slips, trips, falls, contact with other participants, and the effects of

weather, traffic, uneven or loose surfaces, and course conditions. I waive any and all claims I might have based on any of these and

other risks typically found in walking or running in a fun run or race in an outside, uncontrolled environment. I acknowledge all such

risks are known and understood by me.

Release and Indemnity. In exchange for the Hosts allowing me to participate in the Activity, I, on my own behalf and on behalf of

anyone claiming by or through me, hereby RELEASE, INDEMNIFY AND HOLD HARMLESS the Club, the HOA and their respective

officers, directors, agents, contractors, employees, heirs, successors, assigns, volunteers, guests and affiliated entities and agents

(the “Released Parties”) from all liability, claims, demands, causes of action, loss or expense (including court costs and attorneys’

fees) of any kind or nature connected in any way whatsoever to my participation in the Activity, including that which may result,

directly or indirectly, in whole or in part, from the negligence of any Released Party, the negligence or willful misconduct of any third

party, or from the design of the facilities and/or equipment. All personal property brought to the Activity is brought at my sole risk as

to its theft, damage or loss.

Medical. In the event of an illness, injury or medical emergency arising during the Activity, I hereby authorize and give my consent to

Host personnel to secure from any accredited hospital, clinic or physician any treatment deemed necessary for my immediate care. I

agree that I will be fully responsible for payment of any and all medical services and treatment rendered to me, including but not

limited to medical transport, medications, treatment and hospitalization.

Photograph Permission. I give permission for any Host to use, without limitation or obligation, photographs, film footage or tape

recordings that may include my image or voice for purposes of promoting the Club or Host’s programs.

Severability. Any provision or portion of this Agreement found to be invalid by the courts having jurisdiction shall be invalid only with

respect to such provision or portion. The remaining provisions or portion hereof, shall be construed and enforced to the same effect

as if such offending provision or portion thereof had not been contained herein.

Health Statement. I certify as a material condition to my being permitted to participate in the Activity that I am physically fit and

sufficiently trained for the completion of the Activity. To the best of my knowledge and belief, neither I nor anyone in my household

is currently showing symptoms of physical illness. Symptoms of illness are defined as fever, chills, cough, shortness of breath, difficulty

breathing, fatigue, muscle or body aches, headache, loss of taste or smell, sore throat, congestion or runny nose, nausea,

vomiting, or diarrhea.

Minor Participant. If the Participant listed above is a minor, I represent and warrant that I am entering into this Agreement both on

behalf of Participant and on my own behalf and that I have full authority to do so.
